Green Bean Grove is working to bring Food Is Medicine programs to North Alabama. By partnering with clinics and healthcare providers, we can connect patients with healthy food and nutrition support that improves outcomes and reduces the burden of chronic disease.

Why Partner:

  • Improves patient outcomes (A1c, blood pressure, weight management).
  • Reduces missed appointments by tying food pickup to clinic visits.
  • Enhances patient satisfaction (“my clinic helps me eat healthier”).
  • Differentiates your clinic as a leader in preventive, whole-person care.

How Referrals Work:

Green Bean Grove is in the process of building Food is Medicine pilot programs. When launched, partnerships will include:
  1. Formal Agreements: We’ll develop a Memorandum of Understanding (MOU) with each clinic that outlines roles, responsibilities, and sustainability plans.
  2. Funding Alignment: Program costs (food, staffing, materials) will be supported by grants, sponsorships, or clinic investment.
  3. Referral Process: Once agreements and funding are in place, clinic staff will be able to screen patients for nutrition insecurity and chronic disease risk, then refer them using a simple form.
  4. Program Delivery: Patients will receive medically tailored groceries or produce prescriptions alongside nutrition support, with outcomes tracked and shared back to the clinic.

Supports Your Quality Measures:

Food Is Medicine programs help clinics address key measures such as:
  • Diabetes: Hemoglobin A1c control
  • Hypertension: Blood pressure control
  • BMI: Screening and follow up for patients with obesity

These improvements strengthen value-based care contracts, enhance reporting, and improve your clinic’s overall performance.

We are seeking clinic, farm, and community partners to launch a Food Is Medicine pilot in North Alabama. If you are interested in collaborating, reach out today:
